Cold-chain and dose complaints on Trustpilot: a guide for GLP-1 brands

A box of injectable medication sits on a porch in July. The customer opens it warm, calls support, and is told to use it anyway. The review that follows is angry, specific and entirely allowed. This guide covers what a GLP-1 brand can and cannot do about cold-chain and dose complaints. We remove the related reviews that do break a rule, and you pay per removal.

Short answer

Reviews about warm shipments, dose changes and side effects are genuine experiences, and Trustpilot keeps them online. A GLP-1 brand can flag such a review only when it breaks a rule for another reason, for example by naming a pharmacist with contact details or promoting another seller. Replies must not confirm a medication or a dose, and must not make health claims. What reduces these reviews is packaging that holds temperature, a clear replacement policy and support that does not tell customers to use a warm vial.

Two complaints, one cause

Injectable GLP-1 medication is shipped cold and dosed in steps. Both facts create reviews.

Cold chain. The parcel has to stay cold from the pharmacy to the fridge. Heat, weekend delays and missed deliveries break that chain. The customer cannot tell whether the medication is still good, so they ask support. The answer they get decides the review.

Dose. Treatment starts low and steps up. Customers write when a step is refused or when a step makes them ill. They write when a vial holds less than they expected, or when their dose is no longer sold.

In our September 2026 sample, GLP-1 and weight-loss profiles had a low overall 1-star share, about 6% of roughly 178,000 reviews. On single profiles it ran as high as 32%. The totals are in the DTC health Trustpilot report 2026.

Why these reviews stay online

Trustpilot removes reviews for five reasons. None of them is "the customer is wrong about the science". A person who opened a warm box had an experience. A person who felt ill after a dose increase had an experience. Trustpilot says it will not remove a review because a business disagrees with it.

A clinical team may be certain that the medication was still stable. That does not change the rule. The review is about what the customer saw and was told.

Flag by rule, across star ratings. Trustpilot lists flagging a type of content only when it appears in negative reviews as misuse. If 1-star reviews that name your pharmacist are flagged, 5-star reviews that name your pharmacist should be too.

Go slowly. This is the category where a regulator has already described selective challenges to critical reviews as suppression. The GLP-1 page sets out that case.

Replies: three limits at once

A reply on a GLP-1 profile has to pass three tests.

Privacy. Do not confirm that the reviewer is a patient, what they were prescribed or when it shipped. HHS settled a case in 2023 over health information disclosed in replies to reviews.

Health claims. The FTC's Health Products Compliance Guidance defines advertising to include statements on the internet and in social media. Health claims need competent and reliable scientific evidence. "Still effective after 48 hours unrefrigerated" is a health claim.

Tone. Trustpilot removes replies that include personal information, threats or aggressive language, according to its Guidelines for Businesses.

What reduces these reviews

  1. Pack for the worst week of the year, not the average one. Summer weekend delays are when the reviews are written.
  2. Ship early in the week so parcels do not sit in a depot over a weekend.
  3. Write the replacement policy down and publish it. "If it arrives warm, do not use it, and contact us for a replacement" prevents most of the anger.
  4. Never let support give clinical advice. "Use it anyway" is the sentence that appears in the worst reviews. Route the question to the clinical team.
  5. Explain dose steps before the first shipment, including what the customer will see in the vial.
  6. Invite every customer to review at the same point. Trustpilot requires fair and neutral invitations, and recent reviews weigh most.
